Medications at School
Administering medication at school requires written consent from the parent and the student's health care provider for prescription medications. Written parental consent is required for over the counter medication administration. Please use the form below to provide the necessary information to safely administer medications to your child at school. If your child uses medication to manage asthma or severe allergies please use the corresponding forms below for those medications. Medications will only be administered if they are arrive at school in their original packaging and with the required consent forms completed.
