Graduating seniors in Wisconsin are eligible for a Foundation scholarship. Each applicant must be endorsed by a participating local exchange telephone company (Cirrinity). Academic excellence is taken into consideration but is not paramount to receiving one of the scholarships. The primary considerations are: First, the student has expressed a sincere desire to continue his or her education; Second, has demonstrated the ability; and Third, has financial need. |

Applicants:
Must be a second semester senior.
Must have participated in a WIAA sanctioned sport during his/her high school career.
Must have a cumulative GPA of at least 3.5 (on a 4.0 scale). Grades earned through the most recently completed grading period should be used.
Must be involved with civic or community organization(s) or activities during his/her high school career (grades 9-12).
Must be available to attend the Wisconsin Athletic Hall of Fame Induction Ceremony on April 20, 2024. If the ceremony does not take place, Cousins Subs, Inc. will coordinate an alternative event for winners and their family at a date and location yet to be determined.
Must be a college bound student. The educational institution must be accredited, either by the Accrediting Council of Independent Colleges and Schools (ACICS), or by the North Central Association of Colleges and Schools.
Must consent to be photographed, audio recorded and/or video recorded, and all photographs, audio recordings and video recordings become the property of Cousins Subs, Inc. A Publicity Release Form will be required to be signed by all finalists. If the applicant is under the age of 18, an adult must sign on his/her behalf.
